I agree and acknowledge that I am participating workshops, classes, and digital audio/video downloads under my own volition and decision. I understand that instruction provided by Erica Woodland/Woodland Awakening LLC may be physically or emotionally strenuous and I voluntarily participate with full knowledge that there is risk of personal injury, property loss or death. Yoga, meditation, reiki, and shamanic practices are not a substitute for medical attention, examination, diagnosis or treatment. Yoga, meditation, reiki, and shamanic practices are not recommended and are not safe under certain medical conditions. I affirm that I alone am responsible to decide whether to practice yoga, meditation, reiki, or shamanic practices. I agree that neither I, nor my heirs, assigns, or legal representatives will sue or make any other claims of any kind whatsoever against Erica Woodland, Woodland Awakening LLC, her heirs, assigns, or legal representatives for any personal injury, mental distress, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
I acknowledge that it is my responsibility to alert Erica Woodland or Woodland Awakening LLC to any medical issues I may have prior to practicing yoga, meditation, reiki, or shamanic practices. I acknowledge that it is my responsibly to obtain proper medical clearance from my physicians, both physical and mental prior to practicing yoga, meditation, reiki, or shamanic practices.
I acknowledge that all purchase policies have been presented to me and I understand that there are no refunds given for services provided. If Erica Woodland or Woodland Awakening LLC cancels a class or workshop a full refund (less processing fees) will be provided. If you schedule a class or workshop and cancel or do not show up- payment in full is due regardless of attendance. Service credits are not refundable or transferrable.
I acknowledge that by signing this waiver I agree to receive email marketing from Erica Woodland/Woodland Awakening LLC. I have the option to opt out of email campaigns at anytime.
I agree that by submitting this form that I have read, agree, and fully understand the liability waiver presented to me. I understand that I participate in yoga, meditation and any other services offered by Erica Woodland and Woodland Awakening, LLC at my own risk.
